Dr. Donald Polk, D.D.S., died on Saturday, Jan. 5, 2013, in Stuart, Fla. The funeral service is Tuesday, Jan. 8, at 11 a.m. in the Bernheim-Apter-Kreitzman Suburban Funeral Chapel, 68 Old Short Hills Rd., Livingston, N.J., with interment in B'nai Jeshurun Cemetery. Dr. Polk graduated from Seton Hall University with a B.A., Temple University School of Dentistry, and was an alumnus of the L.D. Pankey Institute. His memberships included the American Dental Association, Tri-County Dental Society, Pankey-Mann-Schuyler Study Club of New Jersey (past president), past member of the State of New Jersey Board of Appointed Dental Consultants, the American Academy of Dental Practice Administration, and the American Equilibration Society. Dr. Polk was also a past member of the Academy of Craniomandibular Disorders; past member of the Society of Physiology and Occlusion; fellow in the Academy of General Dentistry, and member and past program chairman of the Newark Dental Club. He presented post-graduate lectures, clinics and seminars to dentists and their staff on restorative dentistry and TMJ disorders. Dr. Polk served in the U.S. Army during the Korean War and was awarded three bronze stars. He was the beloved husband of Rose (nee Vort); the loving father of Douglas, Andrew, and Matthew, and the adoring grandfather of eight.
